Care, Support and Compassion When You Need Them Most

If you or a loved one needs information on health care options to help manage a serious or complex illness, you may benefit from VNA Health Group’s Palliative Care Program which is part of the Advanced Care Institute. Palliative specialists provide comfort care, symptom management, as well as coordination of care and end of life planning for patients with serious illnesses or life limiting conditions. Personalized palliative consultations are provided in the community as well as in facilities.


 
Serious illness has tremendous and far-reaching effects on patients and families. A lack of information, combined with feelings of confusion, anxiety and doubt leaves many families ill-equipped to determine the best course of action. What decisions must be made for you or your loved one – both in the short- and long-term?

 

To provide support and guidance in making these important healthcare decisions, the VNA’s Advanced Care institute offers a Palliative Care Consultation Service. 

 

  • Palliative Care is an individualized approach to addressing the physical, emotional, cultural and spiritual needs of patients and families facing a serious and/or complex illness.
  • The primary goal is to improve the quality of life for the patient and their family.
  • Patients are assessed and provided relief from symptoms such as pain, nausea, or loss of appetite
  • Palliative medicine specialists will also explain the patient’s prognosis, listen to the patient’s and family’s wishes, and develop an appropriate course of treatment.

 

The ACI’s Palliative Care Program focuses on providing palliative consultations in the community as well as in facilities and therefore has built close relationships with local community hospitals as well as skilled nursing and rehab facilities. Collaboration with facility physicians, social workers and other healthcare providers will ensure that all care is well-coordinated and that the patients’ needs are clearly communicated. Palliative consultations help patients and families navigate end of life treatment options.
