HIV Case Management Services

HIV Case Management provides home and community-based services to individuals living with HIV/AIDS. The program connects persons with medical care and community resources. Referrals to community resources include, but are not limited to, housing, substance abuse treatment, and social services.

    Locations

    In Monmouth and Ocean Counties, community-based case management is offered. In Middlesex, Somerset, and Hunterdon Counties, Medical Case Management services are provided.

    Eligibility

    This program is available to individuals in Monmouth, Ocean, Middlesex, Somerset, and Hunterdon Counties who are living with HIV.
