Visiting Nurse Association of Central Jersey
Service Areas: Burlington, Gloucester, Monmouth, Middlesex and Ocean Counties
Visiting Nurse Association of Central Jersey (VNACJ) is most widely recognized for its range of trusted home health, hospice and community-based services that has served the Jersey-shore area since 1912.
Today, VNACJ delivers a wide range of services, all dedicated to ensuring optimal health to patients and their families, across multiple counties with these individual nonprofit business units:
Visiting Nurse Association of Central Jersey Home Care and Hospice
As part of a partnership between VNA Health Group and Barnabas Health, Visiting Nurse Association of Central Jersey (VNACJ) Home Care and Hospice is a 501(c)(3) nonprofit Medicare and Medicaid-certified provider of home care, hospice and palliative care.
Home Health – From visiting nurses to home health aides to nutritional counselors and rehabilitation therapists, VNACJ has been able to keep patients feeling well and cared for within the comfort of their very home.
Palliative Care – The VNA Palliative Care Program offers patients support throughout their entire illness with a dedicated focus on symptom control and pain management.
Hospice Care – To present patients and their families with the highest quality of life, VNACJ offers award-winning compassionate hospice care, allowing for in-home comfort and pain relief through a holistic approach.
Visiting Physician Services
In 2015, VNA Health Group and Visiting Physician Services joined forces to offer an advanced home care model that provides high quality home-based "primary care" health services. House call visits by a doctor, nurse-practitioner or physician assistant, can help relieve the stress and anxiety of transporting a sick loved one to the doctor.
Visiting Physician Services – To better serve patients and expand upon our available services, VNACJ provides quality house call practice at the convenience of patients.
Children and Family Health Institute/Community-Based Services
New Jersey’s most pressing health issues affecting children and families often result from health disparities in access to primary care. This can lead to higher infant mortality rates, inadequate childhood immunization, obesity and chronic illness that carry through the adult years.
The Children and Family Health Institute (CFHI) at VNA Health Group is poised to meet these critical health challenges by building on the agency’s more than 100-year history of caring for children and families and its strong partnerships with public agencies, community-based organizations and health systems.
Children and Family Health Institute – Offering support to families with children with programs such as Pediatric Home Health Care, Women, Infants and Children Supplemental Nutrition Program (WIC) Services in Monmouth and Middlesex counties, Special Child Health and Early Intervention, Home and School based care, Nurse-Family Partnership, and Prenatal and Post-Partum care.
Community-Based Wellness Services – Believing that the first step towards optimal health is education and prevention, VNACJ offers a variety of community wellness services, including immunization programs, HIV/AIDS Assistance, and VNA Community Health Centers.
