Case Management for Long Term Care of the Elderly and Disabled

Our organization provides case management services through several Medicaid Waiver programs. Home and community services are provided for medically and financially eligible persons who are 65 years or older, or for adults of any age who are disabled.

Your assigned VNA case manager is a strong patient advocate and is responsible for assessment of need, coordination, and monitoring of services. Disability management resources can include certified home health aides, lifeline emergency alert system, meal delivery, medical day care, medical transportation, and respite care.

The goals of these programs are to assist individuals in maintaining independence in their home.

Global Options (GO)

The Global Options program is designed to provide in home long-term supportive services, in addition to full Medicaid benefits, to seniors and adults with physical disabilities. The program is designed to supplement assistance already provided by family, friends, and neighbors. Our case managers provide oversight and coordination to each participant.

Community Resources for People with Disabilities Waiver (CRPD)

CRPD is designed to provide services, in addition to full Medicaid benefits, to help individuals remain in their homes and community. Services include case management, environmental/vehicle modifications, community transitional services and personal emergency response monitoring solutions.

Medical and financial criteria must be met to be eligible.

Traumatic Brain Injury (TBI) Waiver

The TBI Waiver helps people between the ages of 21 and 64 who have survived a traumatic brain injury after the age of 16. Services include case management, a structured day program, personal care, community residential services, transportation, respite care and cognitive therapy.
